Registering your Power of Attorney

After downloading and printing your completed LPA, follow the steps below to ensure it is correctly registered and legally valid.

Before posting your application:

  • Carefully review all pages to ensure every field is completed accurately.
  • As the Donor, sign and have witnessed Sections 9 and 15.
  • For a Health & Welfare LPA, also sign Section 5.
  • The Certificate Provider must sign Section 10.
  • All Attorneys must sign and have witnessed Section 11.
  • In Section 14, specify how you will pay the £82 application fee.

Important information

The Donor must always sign first, followed by the Certificate Provider, and finally the Attorneys. This order is essential for validity.

Witnesses must be aged 18 or over. A Donor cannot witness an Attorney’s signature, and an Attorney cannot witness the Donor’s signature.
